JEWISH FEDERATION ACADEMIC SCHOLARSHIP PROGRAM

CHECKLIST:

      Review and verify eligibility requirements

      To begin the process, complete and return, before February 15, 2010, the following forms via e-mail attachments. E-mail to jvsscholarship@jvschicago.org.

    1. Applicant Data Form (New or Renewal)
    2. Career Statement Form (New or Renewal)
    3. Financial Summary

      Complete and return, before February 15, 2009, the following forms via regular mail. Forms can be submitted under separate cover. Mail these forms to: Scholarship Secretary, Jewish Vocational Service, 216 West Jackson Blvd., Suite 700, Chicago, IL 60606.

    1. Legal Domicility Form
    2. Two Letter of Reference Forms (renewal applicants may omit)
    3. Applicant IRS Forms filed for 2008 and 2009 when completed
    4. Parents' and/or spouse's IRS Forms filed for 2008 and 2009 when completed
    5. Documentation of tuition costs, fees and student loans
    6. Release of Information Form
    7. Official Transcripts (If in school, from present school; if not in school, from most recent school attended)

      Your application will not be considered unless ALL documentation is received by February 15, 2010 with the exclusion of 2009 tax returns.

      A JVS counselor will contact you for a personal interview to be scheduled between March 1 and April 20, 2010.
